Why buy at work? You get affordable rates when you buy this policy through your employer, and the premiums are conveniently deducted from your paycheck. You own the policy so you can keep it even if you leave the company or retire. Unum will bill you at home for the same premium amount. Coverage becomes effective on the date you sign the application. 6

Four features that add value Can accumulate cash value at a guaranteed rate of 4.5%*. You could borrow from the cash value or use it to elect a reduced paid-up policy with no more premiums due. Advance benefit option rider is included on all employee and spouse policies of 6,000 or more. You can request an advance, up to 50% of the benefit amount (maximum of 100,000), if you’re diagnosed with a terminal illness limiting life expectancy to six months or less. May vary by state. Adds more coverage that’s affordable – If your employer gives you term life insurance, you have some coverage, but it may not be enough for your needs. You can buy interest-sensitive Whole Life insurance at an affordable premium that is fixed and guaranteed for life. Requires no physical exam – During enrollment, you can get this insurance up to a specified amount without taking a health exam. You may be asked a few health questions. * The interest rate for NJ policies is 5.5%. 7

Additional coverage options Long term care rider Once chosen by the employer, all eligible employees and spouses ages 15 70 with face amounts of 10,000 or more automatically receive the LTC rider. This rider allows the policy owner access to the death benefit after the insured has been receiving long term care for 90 days, subject to rider conditions. The rider can provide a maximum monthly benefit for nursing home care of the lesser of 4% of the death benefit, less any policy debt at the end of the elimination period, or 3,000. The rider also can Provide a maximum monthly benefit for home health care or adult day care of the lesser of 2% of the death benefit, less any policy debt at the end of the elimination period, or 1,500. Provides for a waiver of policy monthly deductions even if the policy does not have the Waiver of Premium Rider. The maximum lifetime value is equal to 100% of the death benefit, less any policy debt. This rider is not available in all states. 8
